3rd Annual SC Population Health Summit

June 8, 2018 @ 8:30 am - 3:00 pm


The summit aims to convene community-based leaders and organizations to help foster community health improvement efforts through a collaborative learning experience. The summit will be held across 12 SC locations, using teleconferencing. Recall, Impact York County stemmed from the 2016 Population Health Summit.

At the completion of this activity, participants will be able to:

  • gain and understand the linkage between the state health improvement plan priority areas and population health.
  • discuss their role at a local level in advancing the work of the state health improvement plan.
  • learn new features and resources in the 2018 County Health Rankings, including a focus on health equity.
  • leave with practical strategies to leverage data to achieve health equity.
  • receive a high-level overview of Healthy Tri-County.
  • learn the steps taken to develop the Tri-County Health Improvement Plan.
  • receive key tools utilized in support of developing the Tri-County Health Improvement Plan.
  • learn strategies to assess community needs.
  • learn innovative approaches to address the social determinants of health through collaboration and partnerships.
  • learn the critical role of a case manager in driving a concept to reality.
  • learn multiple ways to gauge success.
  • leave with practical strategies to achieve health equity and improve population health.
  • share with the group ideas for future opportunities for improving population health and advancing health equity.
